Abstract
Reports a case of possible neuroleptic malignant syndrome associated with
olanzapine in a 40-yr-old female with paranoid schizophrenia
and mild mental retardation. Three weeks into a trial of olanzapine with
doses from 5 mg daily increased up to 10 mg daily, the S fell at home and
when found was incoherent and agitated. Upon initial evaluation in the
emergency department, she was agitated and combative. She was nonverbal, and
her level of consciousness alternated between apparent alertness and
unresponsiveness. On admission, both olanzapine and clozapine, and treatment
with dantrolene along with aggressive iv hydration was initiated. By the 4th
day in hospital the S was alert, cooperative, verbal, and afebrile. ((c) 1999
